Studio Forms

There are a few forms available for you to fill out. Before taking classes at the studio for the first time, and to update after each season, there is the main registration form that must be filled out before your child attends, which asks for the child and parent’s contact information, previous dance school, if applicable, and days and times your child can attend. We ask you to indicate which class/classes your child is interested in and have you sign a release form. We also have a T-shirt and recital book form available to print off and bring with you when needed. A Dancer’s World wants to make this easy for you to send in this information by making the form available online. Fill out the information below and submit it. An instructor from our studio will get back to you within a few days. If you have any questions, don’t hesitate to contact us.

Payment is due the first dance class of each month from September- June. A $10.00 late fee will apply per month the account is past due. NSF Fees for returned checks are $25.00.

I understand that all applied fees and payments are non-refundable.

Drop notification must be submitted by the 1st of the month. After the 1st, all monthly fees apply.

A Dancer's World is released from any liability for any accident or injury occurring on or around the studio's premises or any other locations of the studio's functions.

A Dancer's World may obtain medical services in the event of an emergency.

A Dancer's World may use my child's photo on social media.
